If there is a space provided on the application, you can put them there. Do not put "refences provided upon request" on your resume. It is obvious that the potential employer can request references. What you should do it type out a separate sheet with a list of your references. Make it look nice. Have a heading at the top with your name, address, telephone number. Then list each reference along with their contact information. You may want to include a sentence below each reference describing your relationship with the person and how long you have known the person. Always have these references ready when you to go an interview, and give it to the interviewer if requested.
It should only go on an application where it asks for it. If not submitting an application, you merely say "references provided upon request" at the bottom of your resume. The reason is they do not need it at that point, it is used later in the interviewing process. This also helps your resume to stay short and eye-catching. Good luck!
