A living will usually covers specific directives as to the course of treatment that is to be taken by caregivers, or, in particular, in some cases forbidding treatment and sometimes also food and water, should the principal be unable to give informed consent ("individual health care instruction") due to incapacity.

It does not give power of attorney for making health decisions. That is a separate form.

For the US, you basically need a statement of what you would like to have done if you are permanently unconscious or terminally ill. Then sign it along with a witness.
And make sure people have copies of it, so you don't end up another Terry Schiavo.
